PROGRAM GOALS:

Hazel Street Recovery Center seeks to provide treatment, education and/or rehabilitation services to clients and their families, which promote a drug free lifestyle for optimal functioning within the individual, family, and community by:

  • developing the belief that addiction is a disease
  • developing a state of mental, emotional and spiritual functioning
  • developing a chemical free lifestyle
  • developing and using appropriate methods for dealing with relapse.

PROGRAM OBJECTIVES:

  • Provide an initial screening to all applicants in order to identify the need for treatment
  • After meeting the admission criteria patients will be afforded a comprehensive assessment that considers the special treatment needs of adolescents.
  • Individualized treatment plan is developed for each patient, which includes the need for family participation.
  • Educational programs relating to chemical dependency are offered as a component of treatment and as community service.
  • Qualified credential professionals in the field of chemical dependency provide direct service to patients.

SERVICES PROVIDED

The following services will be provided at Hazel Street Recovery Center:


ASSESSMENTS:

Patients admitted to Hazel Street Recovery Center shall receive an assessment in
each of the following dimensions:

  • A history of the use of alcohol and other drugs that note the age of onset, duration,
  • Patterns and consequences of use.
  • A social assessment.
  • A medical assessment. A patient must provide one at the time of admission or may be referred to an outside source for completion.
  • Diagnostic interview; identifies the symptoms of the disease and the stages of progression.
  • A diagnostic summary that integrates the information obtained from the other components of the assessment.

TREATMENT PLANNING:

A treatment plan is developed for each patient, which takes into account the
special needs of the patient and family identified by the assessment.
 

THERAPY GROUP:

The patients participate in daily Therapy Groups. Therapy Group is a place to share individual assignments, an opportunity to self-disclose, offer peer support and confrontation in a safe environment. The most significant outcome of Group Therapy is the instillation of HOPE in each patient. HOPE keeps patients in therapy, it fosters high expectations, it correlates highly with success. By observing improvement in their peers, each member draws HOPE from the other members. Another strong benefit from Therapy Group is universality; that is to say, a sense of shared problems. Just realizing that they are not unique in their problem is a powerful source of relief. The existential factors of Group Therapy can be summarized in five main areas:

  • Recognizing that life is at times unfair and unjust;
  • Recognizing that ultimately there is no escape from some of life’s pain and from death.
  • Recognizing that no matter how close I get to other people, I still face life alone;
  • Facing the basic issues of my life and death, and thus living my life more honestly and being less caught up in trivialities;
  • Learning that I must take ultimate responsibility for the way I live my life, no matter how much guidance and support I get from others.

INDIVIDUAL COUNSELING:

Each patient participates weekly in individual counseling sessions with his primary counselor. Within the framework of therapy, the counselor becomes the leader for the patient as they both weave their way through the process. No matter how structured your program may be, Hazel Street identifies a need to respond to the individual needs of each patient.
 

RELAPSE PREVENTION/RECOVERY GROUP:

The patients participate in Relapse Group weekly. Patients identify people, situations and places that endanger their recovery, and experience safe environments to practice healthy behaviors. Relapse is a part of recovery and will be present during most people’s early recovery process. We teach our patients that the relapse syndrome causes the individual to become mentally and emotionally inebriated while sober. It begins in a negative pattern of behavior and ends in the return to the usage of chemicals.

FAMILY SESSIONS/PROGRAM:

Patients participate in family sessions with significant family members to open communication and to resolve on-going familial conflict. We understand the areas critical to the development of the patient, is the family of origin. The family system is the basis for the patient’s attitudes and values about himself and the world in which he lives. Our model of treatment supports the theory that the family is a system, and if one or more members of that system are chemically dependent then the system is sick, not just the person who is chemically dependent. The system must be treated, and is best treated as a unit. The family system, when functioning as a unit, is the place where family members should meet many of their needs (the need to be loved, the need for security, and the need to be wanted and needed, among others). If the family does not meet these needs, the system needs repair or redirection.

COMMUNITY GROUP:

Weekly, patients and staff unite in a group setting to discuss and resolve community living issues. A group goal in this process is to demonstrate that being open and honest in your relationships will support your sobriety.

LECTURES:

Patients participate in lectures two (2) times daily, five (5) days a week, during which didactic material relating to chemical dependency and the recovery process is presented. These lectures provide patients with the knowledge and cognitive tools to aid in their recovery.

STRUCTURED RECREATION:

Patients are taken to Legend’s Gym five (5) days a week with staff for supervised recreational activity. A trained professional guides patients through an exercise and weight program.

EDUCATION:

During the patients stay at Hazel Street they are given the opportunity to continue in their educational endeavors, through the Texarkana Independent School District. Hazel Street has an in-house school with a Texas Certified Teacher on property five (5) days a week conducting grade appropriate education with Level I patients. At Level II through Level V, patients attend classes at the TISD-Options campus. Through the utilization of the Options-Self Paced Program, patients can progress at the level of their commitment. We believe the goal of a high school diploma is important, in January and May 2008; Hazel Street graduated five (5) young men. These patients were given the same privilege of any graduate of Texas High School, in cap and gown; they walked and received their diplomas.
 

  • At time of admission, patients and/or parents will sign consent for release of information between Hazel Street and the patient’s prior school(s) of record.
  • Patients will be enrolled in high school through the Texarkana Independent School District-Options which is the local school district area of Hazel Street Recovery Center.
  • Patients will be transferred from their prior school into the Texarkana Independent School District. The Texarkana Independent School District-Options Testing Coordinator, in conjunction with Counselor.
  • Diagnosticians in the Texarkana School District will be responsible for obtaining necessary transcripts from the patient’s prior school(s) of record. (In order to expedite the transfer, a list of forms needed to register is enclosed in the admission, ie, copies of birth certificate, SS card, transcript, report cards, release of records form, etc.)
  • When a patient is discharged from Hazel Street Recovery Center, the Texarkana Independent School District is notified by Hazel Street, and all records will be transferred back to the patient’s prior school upon a formal request of records from that school‘s admission office.
  • Hazel Street Recovery Center supports the TISD-Options, self-paced-modular program approved by the Texas Education Agency. Patients have the option of accomplishing as much as they are committed to completing.
  • Patients are expected to pass schoolwork/participation with a compiled weekly grade of 70%, and to respect teachers as a part of the Hazel Street Staff. Profanity toward staff during school is not acceptable.
  • It is possible for the patient to acquire their high school diploma. Hazel Street has had patients who have graduated from Texas High School. (In 2008, Hazel Street had 5 graduates of Texas High School-Options Campus.)
  • If the patient has their high school diploma, at a Level V, and their treatment plan allows continued stay at Hazel Street, the patient may take college level classes as will coordinate with current treatment schedules
